St. Luke’s Specialist Palliative Care Day Unit is a friendly and relaxed environment located on the ground floor of the hospice. It offers a specialist palliative care service in a daycare setting. The overarching aim is to improve quality of life for people contending with serious illness and their families, using a range of therapeutic and rehabilitative approaches.
Members of the specialist palliative care multidisciplinary team meet with each person to assess their needs and develop an individualised programme of support .The daycare programme is designed to be delivered typically over a 6 – 8 week period, on a designated day of the week (Monday – Wednesday: 9.30 – 3.30pm). The programme is reviewed regularly to ensure it continues to meet the needs of the patient during this timeframe. It is a free service.
The Community palliative care team will continue to support the patient once they have completed their daycare programme.
Families are encouraged to provide transport to and from the hospice but volunteer drivers and a hospital bus are available to those with transport difficulties.
Referral to St. Luke’s Specialist Palliative Care Day Unit is from the Community Palliative Service and/or from Inpatient Palliative Care Services.
