Community Palliative Care Service

Community Palliative Care Service

Community Palliative Care

The aim of the community palliative care service is to support patients who have been referred to the palliative care service at Marymount University Hospice.  The community palliative service comprises of a number of Clinical Nurse Specialists (CNS), led by a community specialist palliative care  coordinator.  The nursing team is supported at the hospice by a medical team, led by Consultants Professor Tony O’Brien and Dr Marie Murphy.  This team is part of the multidisciplinary team which includes Family Support, Physiotherapy and Chaplaincy. There is a satellite team based in Bantry General Hospital which provides a service to the west Cork area, comprising of four clinical nurse specialists. This is led by Dr Patricia Sheahan, Consultant in Palliative Medicine, Kerry & Bantry General Hospitals.

The service is primarily designed to support people who are living at home with serious illness, including those resident in a local community hospital or nursing home.   The service covers all of Cork city & county.

The service is predominantly an advisory one, with any medical decisions being made in consultation with your GP. Clinical Nurse Specialists in palliative care work in close collaboration with your GP and Public Health Nurse to support you and your family at home. Your GP has overall responsibility for your medical care when you are at home.

The CNS provides information, advice and support to patients’ families and health care professionals in the community and is a liaison between the community services and the specialist palliative care team in Marymount University Hospice. The advisory role of the CNS is particularly important in relation to complex symptom assessment and management.

This is a free service and is available from 8.30am-5.00pm; 7 days a week, with a backup telephone advisory service from 5pm to 8.30am from Marymount University Hospice.  To avail of the service a referral form must be sent from either your GP or your Consultant.

Medical referral from your GP or Consultant is essential.
