There is no absolute mathematical formula that may be consistently applied to give a precise ‘equivalent’ dose when converting from one opioid to another. All published ‘conversion ratios’ simply serve to provide a rough indication of the approximate potency equivalence. Thus, it is vitally important to continue to monitor all patients on opioids in order to ensure that we achieve an optimal balance between maximum analgesic effect and minimum adverse effect profile.
